How do I send documents to my doctor?
There are a few different forms you might need to share with us or your doctor.
Photos of your ID
In order to treat you, we need to have a photo of your ID on file. If you haven’t already done so, you can send this using our secure document uploader. Here’s how it works:
- Head to the uploader
- Click “Start”
- Enter your email so we can verify you
- Check your inbox (it may take a few minutes to get your verification email)
- Open the email and click “Verify my email”
- Upload the photo
Our uploader supports jpeg, bmp, heic, tiff, svg, png, txt, and pdf with a maximum file size of 18MB. If you need to send us something in a different format or if you’re having trouble, please email us at hello@talkiatry.com.
Your insurance card
To be sure we’re accurately billing your insurance, we need photos of the front and back of your insurance card. If your plan or insurer changes, send us an updated card and we’ll handle the rest. Here’s how to upload photos of your insurance card:

HIPAA forms
If you need your medical records sent to another entity (doctor, hospital, etc.) or if you need to authorize your clinician at Talkiatry to request your records from another entity, head to our document center. From there, select the option that fits your needs, complete the form, and follow the instructions for how to send it to us.
