Jeffrey Niles, DO

Dr. Niles (he/him) holds the position of Staff Psychiatrist at Talkiatry. He has four years of experience in outpatient psychiatry and is board-certified for adult psychiatry. He has received training in the use of cognitive behavioral therapy, supportive therapy, motivational interviewing, psychodynamic therapy, and dialectical behavioral therapy skills. He uses his training in therapy in tandem with medicine management to provide holistic, person-centered care. During his undergraduate studies at the University of Pittsburgh, while completing his pre-medical classes, he also pursued certification in the history and philosophy of medicine. Additionally, he studied the effect of culture and ethics on healthcare. He earned a Bachelor of Science in Psychology with a minor in Chemistry. He attended medical school at the West Virginia School of Osteopathic Medicine. His psychiatry residency training was completed at Charleston Area Medical Center. While in residency, he worked to improve cultural awareness in the outpatient setting. Dr. Niles is interested in treating patients with a wide variety of diagnoses. He treats mood disorders, anxiety disorders, trauma-related disorders, personality disorders, OCD, comorbid addiction, mild eating disorders, neurodevelopmental disorders such as autism spectrum disorder, and more. He also strives to provide care that affirms LGBTQIA+ identities and underwent additional training in transgender mental healthcare.

Xiomara Nieves Alvarado, MD

Dr. Nieves-Alvarado is a Board-certified psychiatrist who completed medical training at Duke University Psychiatry Program. Originally from Puerto Rico, she is fluent in both English and Spanish, which allows her to connect deeply with a diverse patient population.

Her practice combines medication management with insight-oriented therapy during every visit, promoting a comprehensive approach to mental health care. She advocates for her patients to engage in therapy alongside medication management, as she believes this integrated approach leads to better outcomes and sustained improvement. Dr. Nieves-Alvarado's practice is rooted in a collaborative, team-based approach, working closely with her patients to achieve long-term mental wellness.

Dr. Nieves-Alvarado earned her medical degree from the Universidad Central del Caribe School of Medicine in Bayamon, PR. Afterward, she moved to North Carolina, where she completed her residency at Duke University Hospital. During her training, she was nominated for a fellowship in psychodynamic practice by the Psychoanalytic Center of the Carolinas, showcasing her expertise in this area. She is also a dedicated advocate against childhood sexual trauma.

Her diverse training includes significant experience in jail and forensic psychiatry, as well as the treatment of military veterans under the veteran affairs health care system. She has experience in medically assisted treatments (MAT) for substance use disorders, Electroconvulsive therapy (ECT), and family therapy, enabling her to care for patients with complex needs.

Dr. Nieves-Alvarado treats patients across a wide age range, from young adults to the elderly. Passionate about improving access to mental health care, she joined Talkiatry to help patients facing time or transportation challenges receive the care they need.

Bryce Toburen, MD

Dr. Bryce Toburen is a board-certified psychiatrist dedicated to compassionate, collaborative care across the lifespan. His training includes an MD from the Medical College of Wisconsin, residency at The Ohio State University where he served as Chief Resident, and a fellowship in Child & Adolescent Psychiatry at Nationwide Children's Hospital. Outside the clinic, he enjoys sports, tennis, exploring nature, traveling, and spending time with loved ones.

Each patient brings a unique story, and he strives to create a safe, respectful space where you feel heard and supported. His interest in psychiatry stems from a desire to maximize the well-being of patients and their families through strong therapeutic relationships. Compassion and curiosity guide his work, along with a strong belief in the power of connection and collaboration. Treatment is personalized using evidence-based approaches that include psychotherapy, medication management, and lifestyle strategies. He specializes in addressing anxiety, depression, ADHD, OCD, and trauma-related concerns. Melissa Mott, MD

Staff Psychiatrist

Dr. Mott grew up in southeastern Idaho. She earned a Ph.D. in Molecular Biology from the University of California at Berkeley in 2009, and her Doctor of Medicine (MD) degree in 2014 from the University of Chicago. She completed her psychiatry residency at the University of Utah in 2018 and became board certified in Psychiatry by the American Board of Psychiatry and Neurology in 2019. Following her residency, she worked as an inpatient psychiatrist at the Huntsman Mental Health Institute in Salt Lake City, Utah and served as an assistant clinical professor in the Department of Psychiatry at the University of Utah. In 2021 she moved to Dallas, Texas to work in a residential and partial hospitalization treatment center for eating disorders. She loves to incorporate psychotherapeutic techniques and goal setting together with medication management to help patients achieve wellness and fulfillment in their lives.

Dr. Mott enjoys treating a wide variety of psychiatric conditions, including depression, anxiety, OCD, bipolar disorder, PTSD, eating disorders and substance use disorders. She believes strongly in individualized treatment that focuses on holistic, compassionate and collaborative care with patients.

She loves to incorporate psychotherapeutic techniques and goal setting together with medication management to help patients achieve wellness and fulfillment in their lives.

Lori Peppers, MD

Staff Psychiatrist

Dr. Peppers works with patients to meet the goals that matter to them. She believes that feeling heard and being understood are key factors in a successful treatment relationship.

She has been working in telehealth since 2018. She has worked extensively with patients dealing with trauma from childhood abuse, assaults, combat, and accidents. She also has a lot of experience treating depression, anxiety disorders, bipolar disorder, ADHD, and personality disorders.

After completing her training, Dr. Peppers worked in private practice and in various roles for the Department of Veterans Affairs. She most enjoys working in the clinic because of the long term follow-up.        

Dr. Peppers enjoys her work and includes psychotherapy in her appointments. She offers medication options to treat symptoms but also works with patients to examine behaviors and defenses that may be contributing to their problems. For patients who would benefit from more extensive psychotherapy, she may refer patients to a therapist and work collaboratively with the therapist.

She starts off with a thorough evaluation to make a diagnosis. Then, she works with patients to determine what their treatment goals are and develops a treatment plan to meet those goals.

Jeena John, DO

Staff Psychiatrist

Dr. John's practice focuses on medication management, offered in conjunction with emotionally focused therapy in 30-minute follow-up visits. Occasionally, Dr. John may believe that additional therapy is also needed and ask that you bring a therapist into your care team to provide the best outcome. Dr. John strongly believes in a compassionate, collaborative approach and will provide psychoeducation to help patients make informed decisions and achieve their treatment goals.

Upton Thomas, MD

Dr. Upton Thomas is a board-certified psychiatrist who takes a warm, collaborative, and evidence-based approach to care. With a focus on treating mood disorders, anxiety, OCD, and substance use, he integrates psychopharmacology and psychotherapy to support patients in achieving meaningful change. He brings curiosity, creativity, and a deep respect for each individual’s story to his clinical work.

Patients often find Dr. Thomas easy to talk to and appreciate his calm, thoughtful demeanor. He emphasizes shared decision-making and believes that treatment works best when the patient’s values and voice are at the center. Dr. Thomas aims to create a safe space for honest dialogue and ongoing reflection as treatment evolves.

Dr. Thomas begins with a comprehensive evaluation to understand each patient’s mental health history, current concerns, and goals. He then develops an individualized treatment plan that may include medication, therapy, or both. He prefers to use the lowest effective dose of medications and often integrates psychotherapeutic strategies such as CBT, DBT, or insight-oriented work into his sessions.
